By submitting this form, you (the parent/guardian) agree that you want your child to attend the JYC club for the above selected school and will allow CEF to walk/transport your child (if selected) to the club specified if not located at the school.  If needed, a copy of this form will be given to the school for their records.

Al enviar este formulario, usted (el padre/tutor) acepta que desea que su hijo asista al club de JYC para la escuela seleccionada anteriormente y permitirá que CEF camine/transporte a su hijo (si es seleccionado) al club especificado si no se encuentra en la escuela.  Si es necesario, se entregará una copia de este formulario a la escuela para sus registros.
