All CEF volunteers must sign the Workers Compliance Agreement annually

□    I will follow orders of those in authority over me.

□    I have read and understand the CYIA Dress Code and Code of Conduct and I am willing to cheerfully conform to these standards.

□    I will commit to attending the entire week-long Training School.

□    I have enlisted at least two people to be my prayer partners for my ministry while involved in CYIA.

□    I have read and understand the CYIA Staff Cell Phone Policy (below) and agree to cheerfully comply.

□    I commit to living above reproach in all aspects of my life and setting an example in holy living.

CYIA Staff Cell Phone Policy

Please strictly limit the use of your cell phone and keep it out of sight of students at all times except for navigational use or in case of an emergency. If you must use your cell phone for family issues or for other CYIA purposes that don’t classify as an emergency, please keep it short, away from students, and while on break. Cell phones are not to be used for recreational purposes. You are an example.

I have read the CYIA Staff Agreement, Code of Conduct and Dress Code and will cheerfully conform to it.

Photo & Video Release: I hereby assign and grant to Child Evangelism Fellowship, Inc., its subsidiaries and successors, and assign the unqualified right to the ownership, use and proceeds of all photographs or videos of me, without reservation or limitation, including use of photographs or videos of me for advertising and promotional purposes.
